 Proof of birth (eg. birth record/ clinical booklet/baptismal certificate.
 Certified copy of ID card of parent (s)
 Marriage certificate (where applicable)
 Acknowledgement of paternity (where applicable)
 Certified copy of the ID card of the informant
 Certified Copy of passport (for noncitizens)
