LATE BIRTH REGISTRATION FROM 1 YEAR AND ABOVE.

 Chief’s letter
 Proof of birth (e.g. birth record/ clinical booklet/baptismal certificate/ old passport etc.)
 Certified Copy of ID card of parent(s)
 Certified Copies of passports if parents are (non-citizens)
 Marriage certificate (where applicable).
 Acknowledgement of paternity (where applicable).
 Applicants 18 years and above MUST appear in person before the Registrar.
