 Letters endorsed by the chief from affected families providing reasons acknowledging and supporting the requested change.

 Certified copies of ID cards of at least three witnesses attesting to each family letter.
 Certified copy of the Identity document and birth certificate of the applicant.
 Certified copy of Decree of divorce
