 Chief’s Letter (where applicable)
 Medical certificate of cause of death (MCCoD) (where applicable)
 Certified copy of the mortuary letter (where applicable)
➢ ID Card of deceased.
 Certified copy of the birth certificate when the deceased is below 16 years.
 Certified copy of the ID Card of the Informant
