LATE DEATH REGISTRATION

 Chief’s letter
 Certified copy of Mortuary letter (where applicable)
 Medical Certificate of cause of death – (MCCoD) (where applicable)
 ID Card of the deceased
 Certified copy of Birth certificate where deceased is below 16 years.
 Certified copy of ID Card of Informant.
